INFORMED CONSENT FORM for
Market Impact Identification of Dyadic Attribution Model for Disposition
Assessment Using Online Games (Approved by FSU Human Subject Committee #2014.14243)
This study is funded by National Science Foundation I-CORPS Team (01/01/15-
06/30/15), and is designed to understand how industry members might accept and
adopt a new technology. The research team will survey potential customers from
various industry sectors to understand and identify the market impact of this new
technology.
We are asking you to participate in this research study because you are above 18 years
of age and you are an ideal candidate to give us fruitful feedback on this experiment.
This consent form will explain the study to you.
PURPOSE: Conducting a research study gives us a way to learn more about the
potential market impact of a new technology. In this study, we are trying to learn about
how you perceive this new technology, and how it might be used in your organization.
Barriers to implementation and user acceptance of this technology will be explored
through an interview and a survey.
PROCEDURES: If you decide you want to be part of this study, you will be asked a
series of questions and give us your comments and perceptions about the technology
as presented. After you have been briefed on the product information, you will be
expected to fill out a survey. These activities together should take about 30 minutes.
This is a one-time event. You are welcome to return to the survey if you would like to
participate in the study again, but there is no further obligation.
RISKS: Your personal information and participation will be kept confidential to the
full extent allowed by law. Your personal information will not be identifiable in any way.
We will only report group findings. Thus, there is no risk of disclosing your identity as a
result of participating in this study. There is a small risk of identity disclosure since we
require the email address from each participant, which will be used to distribute the final
report of this study. All identifiable data will be destroyed once the study is complete.
You may withdraw from the study at any time by sending e-mail to isensor@cci.fsu.edu.
BENEFITS: Not everyone who takes part in this study will benefit directly. However,
we think indirect benefits might be available, in that you will learn about a new
technology that is likely to defend your organization against potential human-centric
threats.
COMPENSATION: All participants that complete the interview and survey will
receive a thumb drive. Once the interview and survey has been completed, a thumb
drive will be provided, or shipped.
ALTERNATIVES: Involvement in the study is voluntary, so you may choose to
participate or not. If you do not want to be in this research study, you can let us know.
REPORTS: When we are finished with this study, we will write a report based on
our findings. This report will NOT include your real identity, or that you were in the
study. All information will be kept anonymous and confidential. Data collection does not
allow responses to be connected with any particular subject.
VOLUNTARY: Your participation is voluntary and you may refuse to participate
and withdraw from the study at any time. If you decide to stop after we begin, that’s
okay too. You may withdraw from the study at any time by sending e-mail to
isensor@cci.fsu.edu.
